Whether you need an online survey, a registration form for an event, or other activities, you have a few campus options to choose from: Google Forms, Qualtrics, or you can contact the Office of Planning and Analysis (OPA) to develop more complex surveys, generally those related to capturing institutional data.

Create a new survey on your own or with others at the same time. Choose from a variety of survey types and analyze results in Google Forms.||Qualtrics is a powerful, full-featured web-based platform for creating, sharing and conducting online surveys. Berkeley provides access to all basic features of this software to all campus members.
