Process & Forms

Berkeley IT non-represented staff is required to use the Achieve Online Conversation Tool(link is external).

For represented staff, please continue to use the form approved by the bargaining units: Represented Evaluation 2(link is external). This is an annual form and should cover the review period of July 1 to June 30 of each year. The signed performance evaluations are due to bIT PPC on July 31.
